Lightwire Engineers have isolated the issue to a change made by an upstream provider, which is causing approximately 10% of inbound off-net calls to fail.

Lightwire Engineers have discovered that some inbound calls to Lightwire Voice Systems are failing. The investigation is still on going and we will provide updates as soon as we can.
